   Direct Services Grant Program

Healthcare Georgia Foundation will announce our Notification of Funding Availability (NOFA) for the 2017 Direct Services Grant Program on June 20, 2017. Watch for more details about the NOFA, including the timeline, application materials, and requirements here on the Foundation website.

The Direct Services Grant Program supports existing healthcare services and health promotion programs that assist the Foundation with accomplishing its mission by funding program activities specifically related to the Foundation’s priority area: Addressing Health Disparities. Addressing Health Disparities is the Foundation’s strategic response to unequal access and outcomes among vulnerable populations.

Organizations that have active grants with the Foundation are not eligible to apply for a Direct Services Program Grant during their active grant period.





   Phase I: Community Health Partnership (CHP) and Community Health Improvement Plan (CHIP) 

The purpose of Healthcare Georgia Foundation's Two Georgias Initiative is to provide a place-based grantmaking effort designed to achieve greater health equity among rural Georgians through the elimination of health disparities.

EmpowerHealth  is a two-year pilot grantmaking program that includes a two phase application, starting with the submission of a Letter of Intent (LOI) and later, a full online application. EmpowerHealth replaces the Foundation’s General Operating Grant Program. If you have questions about EmpowerHealth,  please contact Andrea  Kellum, Program Officer, at, phone: (404) 653-0990.
